Job summary
This is an exciting opportunity for a nurse to join our new dynamic Urgent Crises Response Service (UCRS. You will provide rapid specialised assessment and treatment to patients who may present with complex physical and psychological care needs and initiate services to safely avoid unnecessary hospital admission.

The UCRS is an evolving and constantly developing incorporating the following areas: Urgent Community Response (UCR), the cleric ambulance system (working in conjunction with EEAST), virtual wards, in reach into the Integrated Rapid Assessment service (IRAS) based in the Emergency Department Colchester General Hospital, Unplanned Community Nursing and Outpatient Parental Antibiotics Therapy (OPAT) service.

You will travel throughout Northeast Essex treating patients in their own homes therefore a car driver is essential.

You will be proactive in developing the service responding to challenges within practice and also support integration of service, education, practice development and research relevant to the needs of the patients, staff, and students.

Our team is multi-disciplinary and based in Clacton-On-Sea, with other bases in Colchester. Our team delivers high quality care, 08:00 - 20:00 7 day a week within the Northeast Essex community.

Main duties of the job
Provide and receive complex, sensitive or contentious patient related information using tact, persuasion, motivation, negotiation, training and overcoming barriers to communication

Provide specialist professional advice, instruction and/or training to patients, carers, other professional groups and assistants

Communicate effectively with colleagues in the integrated team and across agencies in relevant situations e.g.case conferences, ward rounds and meetings. Analytical & Judgemental Skills.

Make judgements on a range of complex issues requiring analysis, interpretation, comparison of a range of options including information which is reported, not directly observed

Carry out assessment of specialist area, interpreting results to give an appropriate diagnosis and agree the care plan.

Qualifications
Essential

• Registered nurse recorded on NMC register
• Relevant post qualification training through short courses and/or further study and experience for example diagnostics and prescribing, working towards post graduate level of study.
• Evidence of on-going CPD

Desirable

• Recognised registered qualification and further courses and experience relevant to role
• oDiagnostics and Assessment qualification, working towards post graduate level of study.

Experience
Essential

• Professional knowledge and Skills acquired through a combination of formal training, short courses and experience to degree level or equivalent.
• Manage a team
• Act as care coordinator
• Awareness of national guidance relevant to own specialities, including, consent issues, and other legal frameworks relating to care group
• Understanding of the roles of other professionals
• Proven experience working relevant to the care group.
• Supervise the work of students, assistants and volunteers

Desirable

• Competencies relevant to specialism
• Experience of managing the work of others
• Experience in the community
• oConsultation and assessment qualification
